License Application
(a)
An application for a license under this chapter must:(1)
be on a form prescribed and furnished by the department; and(2)
contain a statement made under oath of the applicant’s education, experience, and other qualifications established by the department as required for a license under this chapter.(b)
The department may require additional information regarding the quality, scope, and nature of the experience and competence of the applicant if the department determines that a person’s application lacks sufficient information for consideration by the department.(c)
The department may obtain criminal history record information relating to an applicant for a license under this chapter from the Department of Public Safety and the Federal Bureau of Investigation. The department may deny an application for a license if the applicant fails to provide two complete sets of fingerprints on a form prescribed by the department.(d)
The issuance of a license by the department is conditioned on the receipt by the department of the applicant’s criminal history record information.
