Notification of Rights
(1)
information about the availability of emergency and medical services, if applicable;(2)
information about the rights of crime victims under Subchapter B;(3)
notice that the victim has the right to receive information:(A)
regarding compensation to victims of crime as provided by Chapter 56B (Crime Victims’ Compensation), including information relating to the costs that may be compensated under that chapter and the amount of compensation, eligibility for compensation, and procedures for application for compensation under that chapter;(B)
for a victim of a sexual assault, regarding the payment under Subchapter G for a forensic medical examination; and(C)
providing a referral to available social service agencies that may offer additional assistance;(4)
the name, address, and phone number of the law enforcement agency’s crime victim liaison;(5)
the name, address, and phone number of the victim assistance coordinator of the office of the attorney representing the state; and(6)
the following statement:
