Tex. Occ. Code Section 256.054
Application for Examination


To take the examination under Section 256.055 (License Examination), an applicant for a license must:

(1)

submit an application on a form prescribed by the board;

(2)

pay the application fee set by the board;

(3)

attach to the application:

(A)

proof of current certification in cardiopulmonary resuscitation approved by the American Heart Association or American Red Cross; or

(B)

if the applicant is not physically able to comply with the certification requirements of Paragraph (A), a written statement describing the person’s physical incapacity executed by a licensed physician or an instructor in cardiopulmonary resuscitation approved by the American Heart Association or American Red Cross; and

(4)

provide any other information the board requires to determine the applicant’s qualifications.
