Issuance and Renewal of Certificate


(a)

An applicant for the issuance or renewal of a certificate of registration shall submit to the executive director a sworn application on the form prescribed by the commission accompanied by a $50 fee.

(b)

On receipt of the application and required fee, the executive director shall issue a certificate of registration to the applicant.

(c)

A certificate of registration expires on the first anniversary of the date of issuance and may be renewed annually in the manner prescribed by the commission. An application for renewal must be submitted to the executive director within 30 days before the expiration date of the certificate.
