Tex. Ins. Code Section 1811.052
Use of Approved Certificate of Insurance Forms


(a)

An insurer or an agent may not issue a certificate of insurance unless the form of the certificate:

(1)

has been filed with and approved by the department under Section 1811.101 (Filing and Approval of Forms); or

(2)

is a standard form deemed approved by the department under Section 1811.103 (Standard Certificate of Insurance Forms).

(b)

A person may not execute, issue, or require the issuance of a certificate of insurance for risks located in this state, unless the certificate of insurance form has been filed with and approved by the department.
