Tex. Bus. & Com. Code Section 71.156
Prescribed Forms

(a)

The secretary of state may prescribe a form to be used for filing a certificate or statement that complies with this chapter in the secretary’s office or in the office of any county clerk in this state.

(b)

Unless otherwise specifically provided by law, the use of a form prescribed under this section is not mandatory.
